In an effort to help parents with the schedule of Pennsylvania School Health required physical and dental forms collected by school nurses, here is the list of what is due when.
3rd grade- dental exams
6th grade- physical exams
7th grade- dental exams
Letters and forms are sent home with the students near the end of the previous school year. This allows parents to have exams done and forms filled out over the summer.
When school starts in September, it is helpful if parents send the forms to school in an envelope marked “School Nurse”.
If these exams cause any hardship, the nurse needs to be notified that you would like your child to be examined by the school doctor or dentist. These exams are usually scheduled in March.
